Camphoræ singul: drachmas duas. M: d: t:  1m.d.t. was a standard abbreviation for “misce, da tales” (mix and give [prepared] as such)​​​​​​​

Translation
Volatile Embrocation
Recipe: 6 ounces of camphor spirit, 1 ounce each of sal ammoniac, Thebaic tincture.  2laudanum or Egyptian opium Mix.

Blister Plaster. Pharmacopoeia Londinensis

Pain Relieving Plaster

Recipe. Cumin Plaster. Yellow Wak, one ounce of each. Two drams each of pure opium, camphor. Mix. Give as such.
